Aurora on France is a state of the art senior care and living community, attached to Fairview Southdale Hospital in Edina, MN. It is conveniently located off Hwy. 62 and France Ave. on a public bus line. Aurora is a full continuum of care campus including Transitional Care, Enhanced Care and Assisted Living/Memory Care. Responsibilities/Job Description:- Coordinates admissions with the admissions team as appropriate
- Manages resident caseload with significant turnover to accept/discharge new patients
- Provides one to one and group counseling as appropriate
- Serves as a Discharge Planner
- Assists in Vulnerable Adult investigations and completes necessary documentation and reports.
Bachelor's Degree from an accredited school of social work
Licensed by the MN State Board of Social Work
One to three years experience in LTC
