Overview

Under the supervision of the Data 2 Care (D2C) Program Manager, The HIV Linkage Specialist will play a vital role in Men's Health Foundation's mission to reduce the spread of HIV in the community through our network of express testing and primary care clinics. In the context of rapidly changing and dynamic settings, the HIV Linkage Specialist will strategize, organize, execute, and assess patient outreach, engagement, and re-engagement initiatives to effectively connect individuals living with HIV/AIDS (PLWHA) to essential medical services. Collaborating closely with the D2C Manager, the HIV Linkage Specialist identifies eligible MHF Health patients for D2C services and monitors the outcomes of outreach and engagement endeavors.

The D2C program is a Los Angeles County funded 3-year pilot program to inform future HIV planning and care. The Centers for Disease Control and Prevention (CDC) defines Data to Care (D2C) as a public health approach leveraging HIV surveillance data, pharmacy records, clinic appointments, and various treatment and care sources to reengage clients into care. Its primary aim is to pinpoint individuals with HIV who are not engaged in care, connect them with suitable medical and social support services, and thereby fortify the HIV Care Continuum.

The HIV Linkage Specialist must be able to maintain both strong professional boundaries and a compassionate, sex-positive approach to clients. At MHF, our goal is to educate clients so that they can make the best choices for their own lives.

Essential Functions and Responsibilities
  • Work with key clinic staff in locating out of care patients by conducting outreach services to PLWHA that have fallen out of HIV medical care.
  • Assist in the coordination and delivery of patient-centered activities that focus on addressing health status, engagement, and retention in care.
  • Accurately enter client information into Veradigm, Men's Health Foundation's Electronic Health Record, and Los Angeles County's data tracing system and enter progress notes re: disposition of clients, referrals, changes of address, telephone information, case manager assignment, etc.
  • Coordinate referral and linkage to medical care, Medical Care Coordination, and services that assist with resolving socioeconomic challenges.
  • Collaborate, network, and develop productive relationships with community organizations, and key stakeholders to establish referral linkages.
  • Work independently conducting field investigation (site visits to patient's homes, neighborhoods, shelters, etc.) to locate patients, and refer individuals for necessary medical evaluation and treatment.
  • Develop and implement client identification and engagement strategy;
  • Conduct regular communication with clients to establish rapport and support clients.
  • Document steps taken to help the client maintain access to HIV medical care.
  • Responds to patient inquiries in a prompt manner.
  • Provide HIV education.
  • Implement enhanced surveillance activities, including collaboration with various clinics/hospitals, and medical record abstraction.
  • Assure client confidentiality as defined by MHF policy and procedures.
  • Attends staff meetings, D2C meetings (to review and evaluate program services, and make recommendations for program improvement), case conferencing and other meetings as required.
  • Attend regularly scheduled in-service trainings, as well as outside conferences, and workshops to expand capacity to meet program goals and objectives.
  • Other duties as assigned
Qualifications
  • 2-4 years of public health education or related field experience preferred
  • HIV counseling and testing experience a plus
  • Working knowledge and/or experience working in public health or related field, including understanding of HIV preferred
  • Strong communication skills; works well in a team with a wide variety of people
  • Strong computer skills and ability to input data
  • Good organizational skills and ability to work independently in a fast-paced, diverse environment
  • Understanding of HIPAA and the ability to maintain confidentiality at all times
  • Possess critical thinking ability and problem-solving skills
  • Bilingual (English/Spanish fluency) skills, preferred
  • Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment
