Description

Hospice RN

Summary:
  • Evaluates and admits patients to Hospice Care, develops plan of care and provides care to patient according to plan
  • Provides end of life care and coordination at time of death and bereavement support following death
  • Acts as case manager making necessary adjustments in patient's plan of care related to symptom changes and changes in care needs.
  • Acts as Liaison for patient/caregiver with MD; Community health care partners and other hospice team members.
  • Completes all documentation related to patient contact, assessments, and IDT meetings according to policy
  • Provides on call coverage for both home care and Hospice as assigned
  • Makes necessary Home Care visits as assigned
  • Demonstrates and encourages team behavior and exceptional patient/guest experiences.
  • Upholds and promotes patient safety and quality.
Qualifications

Minimum Knowledge, Skills, Experience Required

Education:

Associates degree in Nursing and currently licensed in the State of Alabama required.

Experience:

Minimal 2 years' experience in a hospital setting working in Med/Surg; Critical Care or ER. Previous Home Care or Hospice experience preferred.

Additional Skills / Abilities:

Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize schedule to meet patient needs.
