Key Responsibilities for Home Health Registered Nurses:

  • Deliver in-home care to patients with various health conditions, injuries, and chronic illnesses, typically post-discharge from medical facilities
  • Collaborate with a team of physicians and supervise LPNs, CNAs, and aides involved in patient care
  • Conduct patient assessments, document progress, and communicate changes to the attending physician
  • Educate patients on their care plan, prescribed medications, therapy, dietary requirements, and physical activities
  • Provide guidance to family members on the patient's care plan, medications, and home safety precautions
  • Ensure treatment goals are achieved and manage the patient's discharge process

Requirements for Home Health Registered Nurses:

  • Active Registered Nurse (RN) License in the state of Colorado
  • Graduate from an accredited nursing school
  • Minimum of 1200 hours of RN experience
  • Current CPR/BLS certification for healthcare professionals (proof of course completion required)
  • Prior experience in home health care is preferred
  • Successful completion of a background check and drug screening (THC testing is mandatory)
  • No mandatory Covid vaccination
