JOB SUMMARY

Under the supervision of the Director of Easter Seals Enterprises and in cooperation with the Easter Seals Enterprises team, serves as a member of the Employment Program team to provide support to adults with developmental disabilities to have real choices in where they live, work and socialize; to train and provide information related to Disability related work benefits; and to promote self-determination in the pursuit of their desired future by personally performing the following duties and other related job duties;

ESSENTIAL DUTIES AND RESPONSIBILITIES

Direct Service
  • Understand and embrace the core concepts of Person-Centered Thinking and the Mission, Vision & Values of the agency.
  • Perform work activities in conformance with Easter Seals Enterprise's values which include Respect, Integrity, Compassion, Stewardship, Learning, Teamwork and Entrepreneurial Spirit.
  • Welcome, value, and embrace cultural diversity in all areas of Easter Seals, i.e., customers, volunteers, employees, individuals served and those that support them.
  • Follow all safety procedures as outlined by the Injury, Illness and Prevention Program Safety Manual and understand the consequences of non-compliance.
  • Provide exceptional customer service both internally and externally.
  • Maintain all needed documentation and records, meeting HIPAA, PHI and other confidentiality requirements as defined by law, the Social Security Administration or Easter Seals policies and procedures.
  • Job coaching to include resume building, job needs, and assessments, placements, and PIP/CIE support.
  • Case Management which includes being involved in and running ISP meetings, case file management and quality assurance and case notes.
  • Teaching on site Vocational Classes in rotation.
  • Benefits support for individuals served to ensure their own personal benefits are not affected by their job/salary.
  • 5 hours per week. Variable days and times as necessary to work with individuals, staff, agency representatives and other stakeholders.
  • Other duties as assigned.
Job Duties

Job Coaching: Provide 1:1 vocational preparation instruction and guidance to individuals with disabilities, helping them to develop their employment experience and opportunities.
  • In cooperation with an individual, their Vocational Support and Contract Operations Managers and the planning team, identify those ready to consider working/volunteering in the community. Provide support in finding potential vocational positions based on their individual needs and preferences.
  • Support an individual through all aspects of the job acquisition process; building a resume, applying for jobs, interviewing, job coaching on site, working with individual and employer to achieve success, and job retention if applicable.
  • Write the Individual Eligibility Evaluation (IEE) and Individual Service Plan (ISP) ensuring that goals set are measurable and applicable to work/skills/training being performed.
  • Complete all required reports and documentation on schedule and submit to required parties. Ensure documentation meets Easter Seals-CARF standards.
  • Provide training and support consistent with an individual's ISP plan, through a variety of platforms including virtual classes, independent study materials and face to face classes/training.
  • Lead relevant and meaningful classes as scheduled (or requested by an individual), by effectively facilitating participation and engagement. Ensure training content remains fresh and meaningful as evaluated by client engagement, attendance, and feedback.
  • Provide curriculum, supplies and written material to individuals and in a form or language they understand to facilitate understanding and successful participation.
  • Assist in identifying needs for accommodations, adaptive equipment, or technology that may increase opportunities for personal satisfaction and employment opportunities.
  • Provide Micro Business information and support in accordance with the individual's personal vocational preferences.
  • Build partnerships and establish relationships with potential employers in various fields to truly provide individuals with options for employment discovery.
  • Transport individuals to work sites when necessary.
  • Learn how to use worksite related equipment as applicable.
Benefits Support: Assisting Social Security Disability Insurance (SSDI) beneficiaries, Supplemental Security Income (SSI) recipients, and recipients of Federal, State, and local benefits to maximize employment.
  • Develop, present and update Benefits Summary and Analysis (BSA) and Work Incentive Plan (WIP) reports for individuals served.
  • Complete required Easter Seals and funding source documentation.
  • Partner with individuals and their Planning Teams to explain available work incentives and how working will benefit the individuals served.
  • Maintain knowledge of various funding sources and the requirements of each with regards to work.
  • Gather critical information about disability related benefits.
  • Provide problem solving and advocacy to individuals and families when appropriate.
  • Facilitate the use of necessary and appropriate work incentives and Provide proactive follow-up services.
  • Possess working knowledge of Employment Network requirements and Ticket to Work Program.
  • Maintain a level of productivity that meets or exceeds budgeted requirements.
  • Other duties as assigned.
Meetings
  • Facilitate, attend, and participate in Individual Service Plan meetings.
  • Attend and participate in all applicable Easter Seals Enterprises staff meetings.
  • Complete all mandatory Easter Seals/Cal-OSHA development trainings annually or as needed.
  • Attend community meetings and/or networking opportunities that can help connect individuals served with potential employers.
  • Other meetings, as assigned.
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/ Training and Experience
  • Associate of Arts degree in a field related to human service, social work, or special education is preferred. May consider an equivalent combination of education and experience.
  • One (1) year full-time experience working with individuals with physical and/or intellectual disabilities.
  • One (1) year case management experience preferred.
  • Obtain/maintain the Social Security Administration's Community Work Incentive Coordinator (CWIC) credential.
  • Obtain/maintain Certification by the Social Security Administration sponsored Work Incentives Planning and Outreach (WIPA) training.
Working Knowledge of:
  • Philosophies, objectives, and goals of community service organizations.
  • Disabilities and related characteristics of individuals served by the agency.
  • Effective interpersonal communication techniques.
  • Community resources, related agencies, and available services.
  • Use of general office equipment.
  • Intermediate computer skills (data entry).
  • Skills required to ensure organization of workload.
Language Skills:
  • Communicate effectively verbally and in writing.
  • Follow written and verbal directions.
  • Effectively be able to present information and respond to questions from supervisor, case managers, clients, and the general public.
Mathematical Skills:
  • Calculate figures and amounts such as addition, subtraction, multiplication, division, fractions, and percentages.
  • Ability to apply basic math concepts.
Reasoning Ability:
  • Ability to solve practical problems and use available resources.
  • Work independently with minimal supervision.
  • Interpret and follow a variety of instructions provided verbally, written, in diagram or schedule form.
  • Maintain composure and good judgment under stressful conditions.
  • Establish and maintain effective working relationships with staff, volunteers, individuals, and the general public.
Additional Certificates, Licenses, Registrations:
  • Position requires possession of a current valid California driver's license, proof of liability insurance and driving record that meets the agency standards.
  • Requires a current certificate in Adult First Aid/CPR with AED from an approved provider.
  • Requires current fingerprint clearance through the Department of Justice.
  • Position requires a pre-placement physical.
  • Position requires proof of negative TB test within the twelve (12) months before hiring.
  • Evidence of full COVID-19 vaccination status as documented by Vaccination Record and includes the passing of two (2) weeks from the last required dose.
