About the Job:

The Clinical Associate plays a crucial role in the interdisciplinary team, working under the guidance of the Clinical Supervisor to deliver exceptional care coordination services to individual patients and groups across all age groups. The main goal is to connect patients with valuable outpatient resources to reduce the chances of return visits.

Working closely with patients, the Clinical Associate acts as a discharge planner, patient advocate, and educator, while also managing case management activities. Using critical thinking skills, they assess and advocate for patients and families to create personalized treatment and aftercare plans, ensuring desired outcomes are met through strategic resource utilization.

To enhance performance, the Clinical Associate undergoes regular individual and group clinical supervision sessions to track progress and maintain competence in assigned duties.


Responsibilities:
- Review and adhere to clinical policies annually.
- Develop and execute discharge plans in line with relevant regulations.
- Coordinate team/family meetings for smooth discharges.
- Stay updated on community resources for patient referrals.
- Ensure patient confidentiality and ethical standards are maintained.
- Assist patients in completing entitlement applications.
- Demonstrate competency in various areas such as effective communication and crisis management.
- Gather and assess patient needs and resources comprehensively.
