Jefferson encourages its employees to expand their knowledge and develop their careers through growth opportunities and continuing education.Responsibilities003848The position will provide behavioral health assessments, brief evidenced-based interventions, clinical/consultative services andeducation using the Primary Care Behavioral Health (PCBH) model, for patients and their family in Jefferson primary care practicesof varying specialties and within the Department of Psychiatry and Human Behavior.
The Behavioral Health Consultant (BHC) in thisrole will also provide consultation with the Primary Care Provider (PCP) to enhance the understanding of the patient, providedecision support for treatment planning, and assist in the ongoing care and monitoring of patients' behavioral health care andbiopsychosocial treatment plans.
Ensures behavioral health services are delivered in a culturally appropriate fashion and isconsistent with State laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines.
Responsibilities :
Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson Provides targeted, real-time assessments and individual, couples and/or family consultations and brief interventions.
Provides effective treatment planning and assisting patients in successfully achieving goals.Assist primary health care providers in recognizing, treating and managing patients with chronic emotional, psychosocial, substance use and/or physical health problems within the primary care setting.
Collaborate with primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
Complete all charting as needed and required by contracted funding sources:including assessments, progress notes, and billing. Triaging and referring patients to specialty mental health care when appropriate.
Participate as a member of the health center staff in the event of any other kind of emergency; other duties as assigned.
QualificationsMasters in Social Work from a School of Social Work accredited by the Council on Social Work Education or an equivalent degree as determined by the Council onSocial Work Education; or PhD/PsyD in Psychology at an accredited university or college.
Licensed Clinical Social Worker (LCSW) or Licensed Clinical Psychologist(PhD; PsyD). Minimum one year of experience in a behavioral health setting.Conditions of EmploymentSince this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check.
