Are you a compassionate and skilled nurse looking to make a difference in patients' lives? We have an exciting opportunity for a Home Health Nurse to provide personalized care in patients' homes.
Position Details:
- Role: Home Health Nurse
- Department: Career Department, Home Health
- Status: Full Time
- Shift: 8:00 am - 4:30 pm
- Average Weekly Hours: 40
Responsibilities:
- Conduct comprehensive assessments of patients and their families to determine home care needs.
- Regularly re-evaluate patients' nursing requirements.
- Develop and implement individualized care plans, revising them as needed.
- Complete accurate documentation, including charting and OASIS reporting.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
- Promote patient independence through education and goal-setting.
- Utilize critical thinking to ensure safe, quality, age-specific care.
- Assist in establishing community resources for patients' well-being.
Requirements:
- Graduate of an accredited nursing program.
- Current licensure to practice professional nursing.
- Experience in home health or related settings preferred.
- Strong communication and organizational skills.
