Specific Dates Needed- Provider needs to be available for all (or vast majority)
- 10/11-10/18
- 10/25-10/27
- 11/25-11/30
- 12/23-12/30
- Schedule: Every fourth week and they would work Monday though Sunday and take call all of those days.
- Work one week out of 4
- Mostly acute care
- Take call when working
- If the provider can do ERPC s that would be a bonus
- Trauma Level/Designation (Hospital): Level 3
- Documentation System/EMR: eCW clinical works OP and Quadramed IP
- Estimated Credentialing Timeframe: 4-6 weeks
- Board Certified
- Active NY license
- Provider within driving distance - Highly preferred
- BLS, ACLS
- Covid-19 Vaccinated
- Available 2 weeks a month with one weekend of coverage- highly preferred
- Must have completed at least 1 successful assignment through presenting agency - Preferred
