Job Summary and Qualifications:
The RN Case Manager is responsible for managing Rehabilitative care and ensuring optimal patient outcomes. Key responsibilities include:

  • Providing program orientation to patients and families.
  • Coordinating patient education and involvement in the treatment plan.
  • Documenting assessments and communicating patient needs.
  • Promoting positive patient interactions and service excellence.

Qualifications:

  • RN with current state licensure; BSN preferred.
  • Minimum of three years' clinical nursing experience.
  • Preferred experience in case management or critical care.
  • Certification in case management or utilization review is a plus.
