Summary
The Clinical Liaison maintains a regular and ongoing presence with SNF and Home Health Agencies to build and maintain strong, trusting, and professional working relationships to improve total post-acute cost of care and improve patient quality and satisfaction.
Minimum Qualifications
Education: Graduate of an accredited school of nursing (Associates of Nursing or Bachelor of Nursing)
Licenses/Certifications: Current, unrestricted licensure to practice as a Registered Nurse
Experience/Knowledge/Skills:
o 1-2 years' experience in a post-acute in a Healthplan or SNF with knowledge of utilization management and CMS criteria.

Tact Medical Staffing Job ID. Pay package is based on 12 hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined. Posted job title: Registered Nurse - Case Management - Hospital
