Responsibilities:

  • Coordinate patient care with their physician and other involved disciplines (PT, OT, ST, HHA).
  • Develop tailored care plans upon admission, set goals, and plan discharges.
  • Adjust care plans as needed based on patient status.
  • Offer direct nursing care and education according to devised plans.
  • Complete assessments and nursing notes promptly and comprehensively.
  • Collaborate with other home health professionals for optimal patient results, attending case conferences for overall care discussion.
  • Supervise HHA and LPN visits when required.
  • Communicate efficiently with team members and supervisors for scheduling and patient updates.
  • Participate in on-call duties as necessary.

Requirements:

  • Graduate of an accredited nursing school with an Associate/Bachelor's degree.
  • Preferably 1 year of home health experience, but willing to train the right candidate.
  • 1-2 years of acute care RN experience is mandatory.
