- Registered Nurse

- Doctor of Pharmacy

Key Responsibilities:
- Conduct comprehensive evaluations for diabetes patients
- Provide direct care and education to patients and families
- Collaborate with healthcare professionals on patient care plans
- Offer support and guidance on diabetes self-management
- Organize and lead group education sessions
- Participate in quality improvement initiatives
- Serve as a resource for patients and families
- Conduct research in collaboration with physicians
- Facilitate referrals to support services
- Ensure compliance with regulations and standards
