Medical Social worker is immediately needed to serve the many psychosocial needs of a cohort of adults with end-stage renal disease in an outpatient hemodialysis located 5 miles East of Downtown Los Angeles.

Masters degree in Social work is mandatory.
Supervision by LCSW is provided

Responsibilities:

  • Conduct initial evaluation and be part of IDT and provide continued assessment per policy and conduct annual depression screening with patients to address specific needs and goals
  • Conduct intake interviews to gather relevant information and assess client needs and learn about and educate patients about kidney transplant procedure and referrals.
  • Provide clinical counseling to individuals and families facing emotional or psychological or social challenges including food and housing insecurity.
  • Utilize psychotherapy and motivational interviewing techniques to support clients in achieving personal growth and positive change.
  • Collaborate with other healthcare professionals to coordinate care for patients with complex needs and adress insurance and transportation issues in a timely manner.
  • Conduct crisis interventions and provide support to individuals in acute distress
  • Advocate for clients' rights and access to appropriate services and resources.
  • Maintain confidentiality and ethical standards in all interactions with clients and colleagues.

Pay:
$ $38.00 per hour

Expected hours:
No more than 40 per week
