Hospice RN Care Transitions Liaison

Position Overview:

  • Serve as a liaison between Aveanna and various healthcare professionals
  • Offer advice and resources to clients to meet their needs
  • Assess services provided and community satisfaction

Essential Responsibilities:

  • Collaborate with physicians, clients, and medical staff to evaluate home care services
  • Communicate with discharge personnel to ensure smooth patient transfer to Aveanna services
  • Educate healthcare professionals about Aveanna services and benefits
  • Identify and establish relationships with potential referring physicians
  • Conduct needs assessments and respond accordingly
  • Attend liaison meetings and assist in enhancing effectiveness
  • Create educational materials and brochures for customers
  • Assist in marketing activities and maintaining referral records
  • Stay informed about the market and competitors
  • Meet or exceed referral and admission quotas
  • Utilize creative means to achieve location goals

Requirements:

  • High School Diploma or GED
  • Minimum of two years healthcare sales experience
  • Active professional clinical license
  • Valid driver's license and automobile insurance
  • Bachelors or Advanced Degree preferred

  • Ability to work independently and in teams
  • Effective problem-solving and decision-making abilities

  • Physical ability to perform job requirements

Environment:

  • Field-based role with autonomy and flexibility

  • Primarily operates outside of a traditional office setting
