**Responsibilities:**

- Facilitating patient referrals to a home health agency
- Collaborating with Physicians and Case Managers to identify patient needs
- Coordinating care transitions between hospital/Physician and the agency
- Communicating with the Executive Director regarding any concerns from Physician offices/Host Hospitals
- Serving as a resource for Care Managers, Physician offices, and patients on Home Health services
- Other duties as assigned by management

**Qualifications:**

- Previous experience in a hospital, Physician's office, Home Health, or Hospice setting preferred
- LPN/LVN or licensed medical professional preferred
- Current Driver's license, vehicle insurance, and access to a dependable vehicle
