Essential Functions

You will:

  • Uphold the mission, vision, and values of apree health
  • Deliver evidence-based primary care to 10 to 15 patients daily, with a focus on preventative care, practical treatment for chronic and acute conditions, and provide disease and case management services
  • Collaborate with patients and your care team, including an onsite Whole Health Coach, to create effective patient care plans that consider each patient's unique needs and diverse backgrounds
  • Ensure comprehensive care aligned with the Vera medical home model, including gathering comprehensive health histories, conducting necessary physical examinations, performing diagnostic tests, and prescribing medications conveniently with onsite lab and pharmacy facilities
  • Assess the healthcare needs of the patient population and engage team members to promote participation in annual health evaluations, health coaching, and other health promotion initiatives
  • Provide clinical leadership by actively participating in team meetings, maintaining clear and proactive communication, prioritizing key tasks, driving process improvements, and supporting change initiatives
  • Meet customer contractual requirements and expectations in collaboration with your care team
  • Participate in rotating schedules with other providers for after-hours coverage
  • Engage in organizational activities and committees to enhance Veras processes and programs
  • Achieve set objectives by improving the health status of the eligible population and meeting benchmarks for clinical outcomes, engagement, and satisfaction
Education & Experience

You possess:

  • MD or DO degree
  • Completion of a Family Medicine or Internal Medicine Residency, with a preference for Family Medicine
  • Current Board Certification by ABMS (ABFM/ABIM) or AOA
  • At least 3 years of experience in primary care
  • Current, valid, and unrestricted medical license along with a valid DEA number
  • Experience in team-based care
  • Proficiency in EMR systems, with a preference for NextGen
  • Knowledge of coding and documentation, including Medicare Advantage and HCC
  • Familiarity with Google Workspace

Compensation: $238k-$350k annual salary (national average, may vary based on markets)
