Job Profile Summary:
- Provide professional and nonprofessional nursing care services as per physician orders.
- Collaborate as part of a diverse team to coordinate patient care, improve outcomes, and enhance efficiencies.
- Oversee projects and programs with specialized knowledge and skills to ensure effective delivery of healthcare services.



Job Overview:
- Utilize case management techniques to deliver comprehensive and coordinated care to patients.
- Conduct clinical assessments, develop personalized care plans, and engage patients through motivational techniques to enhance health outcomes.
- Work closely with the care team to assess, plan, implement, and evaluate patient care in partnership with caregivers and healthcare providers.



Responsibilities:
- Gather patient information for interactions and communicate effectively with healthcare providers and team members.
- Advocate for patient needs, coordinate care, and develop professional relationships with patients and their families.
- Lead multidisciplinary patient care teams, perform assessments, establish goals, and communicate effectively with all stakeholders.



Requirements:
Minimum Qualifications:
1. Completion of a nursing program.
2. Registered Nursing (RN) license.
3. Three (3) years of clinical experience in an acute care setting.
4. Valid driver's license in current state of residence.
Preferred Qualifications:
1. Bachelor's degree.
2. Five (5) years of experience in acute, subacute, or home care, with palliative care and hospice experience being an advantage.
3. Case Management certification and experience.
4. Bilingual proficiency.

1. Familiarity with the target community and a strong sense of connection to it.
2. Exceptional interpersonal, conflict resolution, and communication skills.
3. Ability to work effectively with individuals from diverse backgrounds.
4. Comprehensive understanding of chronic health conditions and population management.
5. Proficiency in Motivational Interviewing techniques.
6. Strong prioritization and problemsolving abilities.
7. Detail-oriented with excellent organizational and multitasking skills.
8. Proficient in technology and business software, including Microsoft Office.
9. Experience with Electronic Medical Records and willingness to adapt to new technologies.



Physical Requirements:
1. Professional office environment with standard office equipment.
2. Position primarily involves sitting but may require movement such as walking, standing, and operating office equipment.
3. Effective communication and information exchange skills.
4. Ability to read computer displays, fine print, and distinguish letters, numbers, and symbols.
5. Occasional lifting and moving objects up to 25 pounds.
