Overview:
The Intake Resource Coordinator is primarily responsible for the intake, triage and referral services within the intake department.

This position, consistent with Family Advocacy Clinician 1 positions, provides general child and family advocacy, interventions, counseling support and prevention services to individuals as appropriate within the intake department at TCFSH.

This clinician works under the supervision of an independently licensed supervisor when relevant, within the scope of their training or licensure, and consistent with hospital policies.

They uphold the fundamental principles and expectations defined in the agency partnership memorandum of understanding, and are accountable for maintaining productive partner relationships.

Responsibilities:
Under the direction of their immediate supervisor, the Intake Resource Coordinator:

  • Answers general questions and makes referrals for families/professionals seeking a range of family violence services.
  • Implements a screening and triage process to assist individuals and/or families in determining the necessary services or assessments. Maintains sound working knowledge of community resources.
  • Conducts crisis intervention and/or safety planning as necessary to increase individual or family functioning.
  • Advocates for clients experiencing family violence.
  • Screens and coordinates clinical services including adult counseling services and multidisciplinary team child abuse investigations through collaboration between all members of the team (law enforcement, prosecution, mental health advocacy, medical assessment, and court advocacy).
  • Major responsibility for gathering referral and pre-admission information from referring agencies, professionals, clients and other customers. Completes all data entry for pre-admission including identifying information, reason for referral, relevant history and payor type information. Assists with related billing functions as appropriate.
  • Identifies emergency or complex cases and seeks clinical review, as needed, prior to finalizing the referral.
  • Maintains ongoing professional development activities required to provide competent services and as defined by relevant licensing authorities, as well as continuing learning expectations required by NCH. Provides documentation of these activities to their supervisor in a timely manner.
  • Demonstrates effective verbal and written communication with peers, supervisors, and partners.
  • Assures confidentiality of clients at all times and consistent with hospital privacy policies.
  • Maintains professional relationships with coworkers, assures a professional and organized work environment, and complies with general hospital policy.

KNOWLEDGE , SKILLS AND ABILITIES REQUIRED

  • Mandated Reporting Requirements.
  • General knowledge of family violence and related concepts.
  • General human development information.
  • Ability to use email to type reports and related documentation, access internet (Microsoft Office).
  • Ability to document in an electronic medical record and related systems. Bachelor's degree in social work, human ecology, child development, education, or a related field. Licensure preferred.
