Job Description: Seeking candidates with minimum 2 years of relevant RN experience in home care. Familiarity with assessment tools such as UAS-CANS-NY or similar systems is preferred. The role involves conducting face-to-face and telephonic assessments for individuals requiring care in homecare/psychiatric/behavioral health settings. Candidates should have strong tech skills, experience in developing Plans of Care, and be detail-oriented.
Responsibilities:
- Developing individualized Plans of Care in collaboration with patients and families
- Performing evaluations and documenting health status and care needs
- Utilizing assessment tools and case management systems
- Ensuring continuity of care and discussing healthcare options with patients
- Collaborating with care team members and maintaining thorough documentation
- Staying updated on community resources and services
- Meeting performance standards and undertaking assigned duties
