Compensation: $17.25 per hour

(Increase to $17.75 after one year of employment.)

Schedule: Monday-Friday 7:30 am - 3:30 pm, No weekends, holidays, or night shifts.

Our Day Habilitation program is seeking a dedicated individual to join us as a key team member responsible for designing, implementing, and overseeing services for program participants. This role involves coordinating the implementation of Service Plans and maintaining individual records.

Key Responsibilities:

  • Set service plan goals, assist in assessment processes, and evaluate potential new program participants.
  • Record daily data, including behavior documentation.
  • Execute service plan objectives and therapeutic milestones.
  • Report any unusual behavior or health concerns promptly.
  • Ensure adherence to schedules and engage individuals in activities.
  • Follow protocols for various aspects like toileting, dining, and safety measures.
  • Maintain communication logs and progress reports.
  • Attend team meetings and contribute to curriculum development.
  • Stay updated on required trainings and implement positive behavior support strategies.
Qualifications:
  • High school diploma or equivalent required.
  • Previous experience in a health care or developmental disabilities setting preferred.
  • Effective communication and teamwork skills.
  • Ability to assist and transport program participants as needed.
  • Experience working with individuals with mental health or developmental challenges.
  • Proficient in service supervision and documentation.
  • Capable of providing physical assistance in emergency situations.
  • Pass required background and driving record checks.
