Key Roles:

  • Welcoming patients into continuous care programs and explaining the range of benefits and services
  • Collaborating with Respiratory Therapists and pulmonary specialists for comprehensive patient care
  • Providing patient education on the use of home monitoring devices
  • Developing personalized care plans through virtual visits
  • Offering coaching on managing chronic pulmonary conditions
  • Regularly reviewing patient readings and updating care plans
  • Being the primary contact for non-emergency patient needs
  • Assisting patients in navigating their healthcare journey
  • Assessing in-home device readings and communicating relevant information to providers
  • Building a trusted relationship with patients to drive positive health outcomes

Candidate Requirements:

  • Active and Unrestricted RN License in Georgia (Compact License preferred)
  • Minimum three years of experience in adult health
  • Background in adult chronic health conditions (Pulmonology, Cardiology, Care Management)
  • Experience with Electronic Medical Records (EMR)
  • Proficiency in basic computer skills
  • Strong problem-solving abilities and multitasking skills
  • Exceptional oral and written communication skills
  • Great teamwork capabilities
  • Clear background check and drug screening
  • Comfortable with remote work and teamwork
