  • Participate in a Medical Home structure to deliver comprehensive care in Southern New Hampshire.
  • Under the guidance of the medical director, practice independently and in cooperation with physicians and other healthcare providers to offer primary healthcare to our patients.
  • Prior experience of 3+ years in a primary care clinical environment.
  • Prior exposure to managing a panel of primary care patients.
