Case load/# patients per day/consults per day 15-20 EMR EPICWill use naviHealth to send referrals and plan to switch to EPIC Care Management over the next yearDo you require InterQual criteria or Milliman experienceNo, but need to have general understanding of discharge readiness Is this position for UR, discharge planning, or bothDischarge planning for complex patient populations, no UR piece to the role.

Potential to plan discharge to a variety of setting including home, home with home health, SNF, LTAC, hospice, etc.

What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.)Will cover all Inpatient units throughout the hospital Currently 15 CM's on the CM team, goal is to have general CM's and 3 Complex CM's).Social Work team consists of 15 full-time and 17 part-time Social workers.

Common types of patients & job dutiesMedically and socially complex patients that require some \"out of the box\" thinking to get them moved.

*In addition to being medically complex discharges patients may have additional needs complicating the discharge such as psych diagnosis, requiring trach and vent management or be sex offenders.

Mississippi is not a Medicaid expansion state so does not cover LTAC or skilled which increases the challenge in placing the complex patients upon discharge.

UMMC has contracts with 5 local facilities with 10 beds reserved at each facility to assist with the complex placements, however specific criteria must still be met.

Criteria has a limitation on financial assistance for only those uninsured or underinsured.

Experience or 'Must have' skillsMust be RN Case manager with 5 years Acute Care Management experience with previous experience in large academic Medical Center hospital UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.

CM Manager/SupervisorDepartment does not have a CM Manager at this time. Will directly report to the Social Work Manager. Chief of Care Coordination is also providing department oversight as needed.
