MDS COORDINATOR (RN LICENSE REQUIRED)

Job Description:
- Maintains and adheres to a schedule of due dates for all MDS.
- Coordinates the completion of the MDS by all disciplines.
- Reviews the MDS data for accuracy and meets with appropriate staff if necessary to ensure precision.
- Completes the MDS and enters it into the system.
- Ensures that assessments are sent and approved by the State promptly.
- Rectifies any rejected records and readies them for resubmission.
- Executes CAAs in line with state guidelines.
- Develops Resident Care Plan (RCP) in collaboration with the interdisciplinary care plan team for all residents based on their needs and within the stipulated timeframe.
- Schedules and conducts RCP meetings regularly and punctually.
- Communicates the RCP to relevant staff members and reports any non-compliance to the supervisor.
- Collaborates with all in-house and ancillary departments to ensure comprehension and adherence to the RCP.
- Carries out other assigned duties as per the instructions of the DON and/or Administrator.
Job Requirements:
- Registered Nurse, a graduate from a recognized School of Nursing, holding a current, valid RN license from the North Carolina Board of Nursing and must provide evidence of license renewal every alternate year.
- Capability to understand, interpret, and comply with personnel, department, and facility policies and procedures, as well as conform to local, state, and federal regulations.
- Proficiency in MDS-RAP and Care Planning functions.
- Preferred experience with RUG-IV.
- Preferred experience with MDS 3.0.
Background checks and drug-free workplace are mandatory.
