Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Provides services as prescribed by the physician in the care plan.
- Offers counseling to patients, caregivers, and families.
- Delivers patient and caregiver education.
Communicates with the physician overseeing the home health plan and other healthcare professionals as needed regarding the current care plan.
Evaluates the patient's functional status, home environment, and equipment needs for a more independent lifestyle.
- Identifies hazards in the home environment.
- Participates in creating a comprehensive care plan.
For individuals aiming to return to work, the Occupational Therapist may conduct work capacity assessments and recommend specialized vocational training.
- Develops treatment programs and sets goals for enhanced functionality.
- Provides instruction on new skills or retraining in daily activities.
- May create splints and educate on adaptive equipment use.
- Guides patients on prosthetic and orthotic device utilization.
Maintains accurate clinical records and communicates any patient condition changes to the physician.
- Participates in quality assessment and improvement activities.
- Identifies additional home health service needs for patients and makes appropriate referrals.
- Trains family, caregivers, and agency personnel on the patient's treatment plan.
- Supervises Certified Occupational Therapy Assistants as per regulations.
