Job Summary and Qualifications

The Nurse Navigator functions as a member of the multidisciplinary team and serves as an advocate and educator for patients with all types of medical conditions.

The Nurse Navigator's primary function is to support patients through complex healthcare environments for the optimization of care and outcomes. The Nurse Navigator facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex diseases, and streamline the intervention process. The Nurse Navigator guides patients and families through the healthcare system by assisting with access issues in an effort to expediate timeliness of care and treatment. Navigators achieve this goal by building relationships with patients and physicians, coordinating the defined plan of care, assisting with appointments, assisting to resolve barriers, providing education, and connecting with community resources. The navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for patients, including support with interdisciplinary and cross-facility coverage and collaboration. The navigator is responsible for timely data collection that promotes tracking of the navigation program performance in line with HCA Healthcare, division and facility goals (growth, quality, practice guidelines, etc). The navigator identifies opportunities to streamline care practice through evidence-based guidelines and ensures quality, patient-focused care.

In this role you will:

  • Serves as a patient advocate
  • Assesses the patient's medical and psychosocial needs as it pertains to their primary medical or surgical condition for navigation.
  • Uses appropriate tools to identify patient needs and barriers to care and provides access to potential resolutions.
  • Identifies health disparities and assists in removal of these disparities.
  • Provides appropriate education, outreach, and support to patients and families. Ensures the patient is empowered to manage his/her own healthcare needs.
  • Provides healthcare system guidance and promotes adherence to recommended treatment throughout the care pathway.
  • Supports providers by assisting patients and/or families in understanding their diagnosis, treatment options, and the resources available, including education, clinical research studies (as applicable), and technologic advances.
  • Provides education/connection to community and external resources, such as access to supportive care, financial support, and return to work.
  • Streamlines processes for patients by assisting with appointment scheduling in a compliant manner.
  • Ensures the organization of appointments and explains the sequence of treatments.
  • Ensures smooth transitions between care modalities, facilities, and providers.
  • Facilitates patient movement through the appropriate clinical pathway and collaborates with physicians to ensure patient compliance.
  • Reinforces patient-specific teaching as defined by the patient's provider.
  • Orients patient to hospital/provider logistics, if appropriate.
  • Conducts follow-up conversations with patients and communicates any concerns, changes, or social needs to the appropriate physician or other care provider.
  • Attends and participates in Multidisciplinary Meetings (MDM) and/or other care planning conferences, as required by navigation program.
  • Ensures appropriate patient data is available, including assessment and documentation.
  • Serves as a patient advocate and multidisciplinary team member at these meetings.

  • Educates the patient and family regarding planned procedures, medications and treatments.
  • Drives process improvement for navigation operation standards and efficiencies to provide better care to patients.
  • Provides referring physicians ongoing status for navigated cases, including communication when navigation has concluded.

Qualifications:

  • Bachelors of Science in Nursing (BSN) from an accredited school of nursing preferred.
  • Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation. – Required
  • American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification.
  • Minimum of 3 years of obstetrical nursing experience required. Previous experience in program development, nurse navigator and / or patient educator in childbirth preferred.
  • Experience in leadership and customer service required.
  • RNC-OB, RNC-NIC, RNC-MNN, IBCLC or professional certification (Preferred).
