Role Responsibilities
- Coordinate cost-efficient, reimbursable patient care under the insurance plan and physician approval
- Verify insurance information and ensure services align with the care plan
- Oversee compliance in clinical documentation, care coordination, and billing processes
- Participate in Quality Improvement activities and maintain high care standards
- Supervise pediatric nurses and support their professional development
- Effective communication with the interdisciplinary team to optimize patient care
- Facilitate patient discharge planning and ensure thorough documentation of care activities
Requirements
- Licensed and registered as a Registered Professional Nurse in New York State
- BLS certification required
- 3+ years of experience in home care/CHHA environment, supervisory experience preferred
- Prior experience with pediatric care and full COVID-19 vaccination
- Reliable transportation within Brooklyn, Queens, and Manhattan, ability to work holidays and weekends
