Primary Care Nurse Practitioner-Serious Mental Illness

Our company is focused on revolutionizing the way individuals with Serious Mental Illness (SMI) are cared for. We are on the lookout for a caring and skilled Nurse Practitioner to take charge of the medical aspects of our team-based care for individuals with serious mental illness. This position is based in Tacoma, WA.
Responsibilities:- Conduct thorough whole-person clinical assessments including gathering information from various sources
- Evaluate both behavioral and physical health conditions through detailed history and physical examinations
- Identify undiagnosed conditions and order necessary additional studies
- Organize information to assist with successful disability applications if needed
- Perform field visits or home visits when required
- Lead the medical segment of team-based care by training and guiding our team on clinical issues
- Support care coordination by identifying gaps and managing medication reconciliation
- Communicate effectively with individuals' care providers from various healthcare settings
- Display empathy, compassion, and approachability
- Possess excellent listening, communication, and interpersonal skills
- Have a genuine desire to aid those in need and guide them towards wellness
- Enjoy working collaboratively in a diverse team with varied responsibilities
- Show cultural humility, knowledge of trauma-informed care, and understanding of social determinants of health
- Be comfortable with data-driven care and have knowledge of local healthcare providers
- Capable of maintaining professional boundaries and engaging effectively with a challenging population in unconventional work environments
- Intermediate computer skills with familiarity in Microsoft Office Suite
- Hold a driver's license, private auto insurance, and be insurable under company coverage
- Minimum 3 years of clinical experience as a Nurse Practitioner, preferably in a primary care setting
- Experience working with underserved populations facing behavioral health and/or SUD challenges
- Background in developing and delivering educational programs for medical staff
- Proficiency in care management and coordination
