WHAT YOU WILL BE DOING (ESSENTIAL DUTIES)

Essential Duties:

1.

Development of Community Partnerships:


  • Identify and establish strategic partnerships with community organizations, healthcare providers, social services, and other stakeholders.
  • Maintain and strengthen existing relationships to enhance service delivery and support for clients.
  • Represent the CCBHC at community events, meetings, and forums to promote the center's mission and services.

Creating Protocols for Care Coordination:


  • Develop, implement, and update protocols to ensure seamless coordination of care among different service providers.
  • Collaborate with clinical and administrative staff to ensure adherence to care coordination protocols.
  • Provide training and support to staff on new protocols and best practices in care coordination.

3.

Developing and Tracking Care Coordination Contracts:


  • Draft and negotiate care coordination contracts with community partners and service providers.
  • Monitor and evaluate the performance of contracted services to ensure compliance and quality of care.
  • Maintain accurate records and documentation related to all care coordination agreements.

4.

Re-Entry Liaison from Psychiatric Hospitalizations:


  • Serve as a point of contact for clients transitioning from psychiatric hospitals back into the community.
  • Develop individualized reentry plans in collaboration with hospital staff, clients, and their families.
  • Coordinate followup care and support services to ensure a smooth transition and continuity of care.

Re-Entry Liaison from the Judicial System:


  • Work with judicial system personnel to facilitate the reentry of individuals from correctional facilities into communitybased behavioral health services.
  • Develop reentry protocols and support plans tailored to the needs of clients involved in the judicial system.
  • Monitor and provide ongoing support to ensure successful reintegration and reduce recidivism.

WHAT WE EXPECT OF YOU

  • Bachelor's degree in Social Work, Psychology, Public Health, or a related field (Master's degree preferred).
  • Minimum of 3 years of experience in community relations, care coordination, or a related role within a behavioral health setting.
  • Strong understanding of behavioral health systems, community resources, and care coordination practices.
  • Excellent communication, negotiation, and interpersonal skills.
  • Ability to work collaboratively with diverse groups and manage multiple projects simultaneously.
  • Proficiency in data management and reporting.
  • Familiarity with regulatory requirements and best practices in behavioral health care.
