Job Description:

This opportunity involves working in a Residency Clinic setting with a focus on offering direct patient care while also providing education to Providers in the field of Behavioral Health.


Overview:

A Clinical Behavioral Health Consultant plays a key role in the healthcare team, supporting care managers in overseeing the health of their patient population.

The main objectives of the role include enhancing the identification, treatment, and handling of psychosocial/behavioral issues and medical conditions within the patient population.

Furthermore, the consultant delivers mental health consultation services within the parameters defined by the state's licensure board to all patients referred by the care team.

The role also involves managing psychosocial aspects of chronic and acute illnesses, applying behavioral strategies to tackle lifestyle and health risks, and collaborating in the treatment of mental health disorders.


The consultant serves as the primary resource for mental health and substance abuse assessment, intervention, and provision of higher levels of care when needed.


Key Responsibilities:

  • Member of Care Team & Population Health Initiatives
  • Contributes as a core team member to establish specific clinical pathways or programs for targeted patient groups
  • Participates in practice meetings and staff training sessions
  • Offers education and assistance on various topics related to behavioral health and substance abuse to team members during patient treatment planning
  • Engages in daily huddles to identify behavioral health, substance abuse, and psychosocial requirements
  • Delivers care based on registry to high-risk patients
  • Extends services to all enrollees, not solely those with diagnosable mental disorders
  • Engages in a system-wide Behavioral Health Consultant learning collaborative
  • Efficient Communicator and Catalyst for Change
  • Advocates for continuous improvements within the practice and the broader health system concerning mental health and substance abuse issues
  • Documents within the same medical records as other team members for easy visibility by all
  • Communicates with patients via phone and in-person
  • Clinical Interventions
  • Meets patients and providers promptly, often on the same day a patient arrives, to provide immediate support through assessment, intervention, and referrals for higher-level mental health and addiction care when necessary
  • Provides counseling services suitable for ambulatory care settings, utilizing short-term interventions
  • Collaborates with patients' families and support systems
  • Facilitates brief follow-ups, including education on relapse prevention
  • Supports patient skill-building for improved psychological and physical health, including group sessions
  • Assists patients in crisis to access urgent mental health and substance abuse services
  • Triages and refers patients to specialized mental health and substance abuse services appropriately
  • Advocacy, Community Engagement, and Coaching
  • Champions for patients facing challenges or appearing 'non-compliant'
  • Re-engages patients not actively participating in their care
  • Encourages patients to take an active role in their care and well-being
  • Connects patients with community resources related to social determinants of health

Requirements:

  • Master's degree in a Mental Health Field
  • 2 years of experience in evidence-based counseling techniques and handling common mental health and substance abuse disorders
  • Understanding of the interplay between medical and psychological aspects of health
  • Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) - candidates with out-of-state licenses have 60 days to transfer upon hiring
