Social Caseworker I

ESSENTIAL DUTIES:
(The following duty statements are illustrative of the essential functions of the job and do not include other non-essential or marginal duties that may be required. Mesa County reserves the right to modify or change the duties or essential functions of this job at any time. All responsibilities may not be performed by all incumbents.)
Provides one-on-one assessments of at-risk adults who are unable to protect their own interests based upon the following: a) physical appearance and hygiene; b) medical, nutritional, and environmental needs; c) competency and ability to understand self-determination and the result of the situation that is placing him/her at risk; d) activities of daily living; 3) support systems; f) ability to manage financial affairs.
Seeks court action to obtain protective services and/or potential guardianships in emergency situations.
Works closely with law enforcement agencies and district attorney s office in investigations of abuse, neglect, and exploitation of at-risk elders.
Works collaboratively with multiple community agencies to assess, investigate, and implement services for at-risk adults.
Performs case planning for individuals who require ongoing protective services which may include referral for services to reduce harm and risk.
May attend interdisciplinary team meetings regarding client care, advocacy, and service coordination; attends conferences and meetings of various types to accrue mandatory continuing education hours.
Documents assessments and actions taken throughout all phases of the case.
Oral and written communication must be clear.
Performs other related duties as assigned.
Minimum Requirements MINIMUM QUALIFICATIONS REQUIRED:
A bachelor s degree from an accredited institution with a major in a human behavior science field,
OR
a degree with 30 semester hours, or 45 quarter hours, of qualified work in development in human behavioral sciences or health care related courses, such as, social work, sociology, psychology, psychiatry, gerontology, nursing special education, family intervention techniques, diagnostic measures, therapeutic techniques, guidance and counseling, criminal justice, or other human behavioral sciences, or a
medical field relevant to the APS program and/or at-risk adults.
AND
One year of professional casework in a public or private social services agency completed after the degree is obtained.
Experience working with adults is preferred.
Supplemental Information PHYSICAL REQUIREMENTS
AND WORKING ENVIRONMENT:
This position requires standing, stooping, sitting, bending, twisting, and lifting up to 10 pounds.
Work is generally confined to a standard office environment.
Employee may exposed to disease of known or unknown origin, blood borne pathogens, body fluids and tissue, the threat of indirect violence/conflict by other individual, and other unanticipated circumstances associated with the position.
