- You will deliver psychosocial support and Social Work services to Hospice patients and their families across various settings, including inpatient facilities, home care, and nursing homes.
- Acting as a key member of the hospice IDG, you will facilitate communication between hospice teams and inpatient/nursing home staff.
- Developing personalized care plans and goals, you will provide recommendations based on the unique needs of patients and families under your care.
- Create discharge plans for patients admitted to inpatient facilities when appropriate.
We are looking for individuals with a passion for social work and a desire to make a difference in healthcare.
- Graduation from an accredited School of Social Work is required.
- Minimum of one year of experience in Social Work within a healthcare setting.
- Proficiency in Word and Excel is preferred.
- Hold a valid New York State LMSW certification.
- Possess a valid NYS Driver's license and access to a car.
- Capable of collaborating effectively in an interdisciplinary team environment.
- Demonstrate proficiency in entering clinical data accurately.
- Fluency in both English and Spanish is a plus.
