Minimum of 2 years experience in Case Management or Discharge Planning in an Acute Care setting required. Experience working in a Critical Access Hospital or rural health setting highly preferred.

screen patients for discharge planning needs and assists patients with arrangements; participates in infection control surveillance; prepares a variety of reports assigned by Chief Quality Officer; participates in infection surveillance and patient and staff education related to isolation precautions; actively participates in Quality Management and Hospital Safety Programs.

Shift: 8 hour days x 5 days per week, including some weekends. Rotating call, rotating weekends, and rotating holidays.

Orientation:
Held every Wednesday, consisting of three days of classroom orientation and one shift with preceptor.
