About the Company

Cornerstone Service Center, a division of Pennant Services, seeks a committed and passionate Clinical Documentation and Education Specialist to join an organization dedicated to providing life changing home health and hospice services. This position offers a unique opportunity to support our clinical leaders at agencies located throughout the Western United States. In association with those local clinical leaders, the successful applicant will be responsible for designing, monitoring, implementing, and evaluating process and programs to meet regulatory and accreditation requirements and to achieve the highest standard of care delivery system.

About the Opportunity

The Clinical Documentation and Education Specialist is a Registered Nurse who is responsible for designing, monitoring, implementing, and evaluating documentation review and education programs to meet regulatory and accreditation requirements and to achieve the highest standards of care.

Duties and Responsibilities

Performs Eligibility and/or Quality Assurance audits in a proactive, recurrent manner to support Portfolio partners. Works under the direction of the clusters and Clinical Market Leaders on identified problems and deficiency resolution. Reviews documentation specific to Length of Stay and make recommendations accordingly. Reviews documentation related to Medicare eligibility requirements and State/CMS regulatory guidelines and provides written and verbal report of findings. Identifies educational needs based on documentation assessment and conducts or coordinates in-services as appropriate. Utilizes information gathered to educate on best practices and improved processes. Guides local management with appropriate discharge plans and timeline when indicated. Guides local management on improving the quality of patient documentation from all disciplines to support terminality. Assists with design, periodical review, and implementation of Quality Assurance Plans. Stays current on regulatory requirements and standards for both home health and hospice. Keeps abreast of all industry best practices and CMS guidelines. Informs local leadership on areas of risk identified during documentation reviews related to regulatory and legal concerns. Coordinates and supervises local resources with performing supplemental documentation review and providing onsite education related to findings. Tracks and trends findings to identify more agency/market/clinician specific education needs.

Qualifications
  • Licensed Registered Nurse
  • Minimum 3 years Home Health or Hospice experience with one year supervisory and educator experience preferred.
  • Understanding of the legal and regulatory framework governing the home health/hospice industry
  • Ability to work with all levels of management on all aspects of clinical operations
  • Verbal and written communication skills necessary to explain complex and/or confidential information.
  • Time management and organizational skills necessary to manage multiple projects, appropriately prioritize workload, plan for agency leaders to meet deadlines and goals, and work independently in responding to day-to-day functional needs.
  • Demonstrated ability to interact effectively with employees.
  • Ability to anticipate and identify problems and use sound judgment and fact-based analysis to develop effective and efficient solutions.
  • Software skills, including use of Microsoft Office software and web-based applications.
