OPS ONLY Duties and Responsibilities:


The primary responsibility of this position is to a ssist in the coordination and documentation of health education messaging, community outreach events and activities as outlined in the CHP workplan.

Assist in the coordination and documentation of outreach events and community spotlights submissions.

Disseminate information to the appropriate partners regarding, resources, events, grant opportunities, health care access and quality, best practices, etc. monthly.

Maintain documentation of requested and disseminated resources.

Attend and participate in the meetings and coalitions that serve the community.

Increase the number of community partners and/or collaborations in projects.

Update Coalition members on CHD health messaging projects, events, issues, partner recruitment, and data tracking.

Maintain a professional and effective working relationship with external organizations that address health disparities.

Maintain resource guide of local agencies and organizations .

Update FDOH-Nassau Community Health SharePoint Events Calendar

Conduct planning and preparation for our reach events. Prepare weekly outreach after action reports.

Attend conference calls, trainings, and webinars regularly for professional development.

Lead or co-lead at least 4 quarterly (CHD internal) meetings, activities or events.

Coordinate/participate in activities/events that support the improvement of resident's health status.

Track priority area projects goals, objectives, activities in the FDOH ClearPoint tracking system and update data monthly.


Serve on one (1) internal committee such as Employee Wellness Committee, Workforce Development, Performance Management Council, Customer satisfaction or Safety committees.
