Weber Human Services (WHS), located in Ogden, Utah serves a growing urban population residing in Weber and Morgan Counties.
We are an organization dedicated to helping those in need rebuild their lives and reach their full potential.

WHS was organized by a cooperative agreement between the Weber and Morgan county governments to serve as the local authority for the provision of their substance abuse, mental health and senior service programs.

Weber Human Services is the only organization in the State of Utah providing these services under a single administration.

Through dozens of human services programs, WHS helps thousands of its neighbors by supporting and empowering our most vulnerable groups including at-risk youth, the elderly, children, youth and adults suffering from mental illnesses, and those recovering from addictions.

This is a grant funded (COSSUP) position providing case management services within the Weber County Correctional Facility.

The purpose of this position is to assist individuals with accessing services efficiently and decrease gaps between service agencies when individuals reenter the community.

Supervision of the position is shared between Weber Human Services and the Weber County Sheriff's Office.

The position is part of an interdisciplinary team that works closely with partner agencies to facilitate case coordination and successful transition into the community that may include shelters, housing authorities, and landlords to secure and maintain permanent housing.

The population served includes individuals with substance use disorders and co-occurring disorders who are moderate to high risk for recidivism.

Responsibilities include providing resource education and individual case management services to individuals while they are incarcerated and upon reentry.

Targeted case management duties include providing needs assessment, screenings, linking individuals to community resources prior to reentry, outreach, advocating with and on behalf of clients, and working closely with community partner agencies.

Documentation requirements include data entry in various case management systems for tracking and outcome documentation. Case plan management includes shared goal development with a client, monitoring progress, and follow up with case closures. Expectations also include attending trainings, team meetings, and supervision. Other duties may be assignedHigh school graduate/equivalent. Certified Case Manager (or must obtain certification through Agency within 1 month of employment). Preferred but not required Bachelor Degree in social work, criminal justice, or related field. Further preference for SSW licensure. If SSW licensed, must maintain license by meeting all requirements of licensure including obtaining requisite CEU's.

Must have a working understanding and experience working with adults with substance use disorders, co-occurring disorders, individuals involved in the criminal justice system and may be experiencing homelessness.

Must have awareness of available community resources, effective communication skills both verbal and written, collaboration skills, ability to work in a team environment, excellent computer skills.

Valid Utah driver license with good driving record (no more than 1 violation) since this job will require driving for the agency.

Must be willing to complete and criminal background check and pre-employment drug screening.
