Do you wish to have a positive impact in your local community? The In Home Caregiver role provides tailored support to individuals with disabilities, aging individuals, or those facing mental health challenges, assisting them in their journey towards greater independence. This position, spanning 15-25 hours weekly, is part of the Community Supported Living team (CSL) which focuses on empowering clients to enhance their independence and continue living in their homes within the community.



Main Responsibilities:
  • Encourage personal growth and life experiences determined by the individual.
  • Collaborate with and assist the individual in discovering opportunities for involvement and relationships within the community.
  • Support the individual in expanding their network of support and organizing support group meetings.
  • Actively engage as a member of the individual's support network.
  • Guide and educate the individual on leveraging community resources.
  • Foster an independent lifestyle through ongoing skill development.
  • Promote optimal health by providing guidance on personal hygiene and other health-related needs.
  • Identify and address safety concerns in collaboration with the individual.
  • Resolve safety issues related to the individual receiving services and take necessary action.
  • Complete documentation and reporting for clients, funders, and/or LSS promptly.
  • Participate in LSS meetings as required.
  • Attend training sessions as assigned and seek relevant additional training opportunities.
  • Embrace, advocate, encourage, and manage change effectively.
  • Commit to empowering the individual to take charge of their own life.
  • Initiate collaborative problem-solving with the individual.
  • Assist individuals in making informed decisions.
  • Handle crisis situations efficiently using both internal and external resources.
  • Mediate conflicts when necessary.
  • Adhere to directions, respect boundaries, and meet productivity standards set for the role and program.
  • Undertake other duties and special projects as needed.

Qualifications:
  • High school diploma or GED equivalent preferred.
  • Valid driver's license and reliable transportation required.
  • First Aid and CPR certifications preferred.
  • Proficient in reading, writing, and analyzing to complete necessary documentation on time.
  • Skilled in presenting information effectively to various audiences.

Essential Computer Skills:
  • Proficient in using computers for tasks such as time card completion, email correspondence, and online training.
  • Adept at utilizing electronic health records for service documentation.
  • Capable of working with different software and computer systems.
  • Familiarity with supportive technology and adaptive equipment for individual support.

Physical Demands and Work Environment:
  • Must meet physical requirements specified for the service site.
  • May involve standing or walking for extended periods.
  • Expected to perform various physical activities, including bending, stooping, reaching, and lifting objects.
  • Being prepared to handle crises, aggression, or emergencies effectively.
  • Exposure to household allergens and outdoor weather conditions.
  • Travel to different community locations may be necessary.
