MUST POSSESS A MASTER'S DEGREE IN A CLINICAL MENTAL HEALTH DISCIPLINE SUCH AS PSYCHOLOGY, COUNSELING, CLINICAL SOCIAL WORK, PSYCHIATRIC NURSING, OR MARRIAGE AND FAMILY THERAPY TO BE CONSIDERED FOR THIS JOB.

SUMMARY:

The Violence Intervention Mental Health Professional will assist the community outreach team with training and support in the Rewire CBT Model and by providing therapy to program participants, as needed. They will also train parents in one of APM's two Evidence Based Child Rearing programs, one for the parents of children ages 1-10, and the other for the parents of youth (Raising Safe Kids and Strong African-American Families). The Violence Intervention therapist will perform community outreach based on referrals received from community contacts about persons victims of non-fatal shootings and persons at risk of perpetuating a cycle of violence in North Philadelphia and engage those persons together with APM's Violence Intervention Community Connectors.

DUTIES AND RESPONSIBILITIES:

  • In collaboration with the APM Violence Intervention team, provide community engagement and in the 24th and 26th Philadelphia Police Districts under the direction of the Program Director and community leadership.
  • Outreach to non-fatal shooting victims as identified by Police Department, ideally within 48hrs of an incident occurring, together with the Community Connectors;
  • Working with Hospital Based Violence Programs to engage with and deem eligibility for non-fatal shooting victims in their care, together with the Community Connectors;
  • Represent APM as appropriate in community events, meetings, learning communities in coordination with other APM Program Staff;
  • Support Community Outreach Teams with Rewire CBT training and interventions.
  • Collaborate with APM Directors to design plans to engage young people on a weekly basis. This will include off-track young people, red flags who have been difficult to locate, young people who have not been attending programming and those who may need intentional conversations around safety issues;
  • Train parents in age-appropriate Evidence Based Parenting program.
  • Provide direct day-to-day CBT and / or PE therapeutic services to individuals and families, as needed.
  • Evaluate and treat mental illness through assessment and diagnosis.
  • Develop and implement comprehensive therapeutic treatment plans that reduce or eliminate specific symptoms associated with the diagnosis, along with other challenges and barriers that impede daily life functioning.
  • Facilitate interventions to improve functioning as well as encourage and empower individuals using evidenced based practice therapeutic approaches.
  • Assist individuals with accessing ancillary resources by coordinating support services through advocacy, linkages with community resources and APM internal available services, together with Violence Intervention Case Managers.
  • Maintain accurate clinical documentation including but not limited to assessments/evaluations, individual, family and group therapy session progress notes, treatment plans, and consent forms, APM intake package, client notes, and outreach forms.
  • Coordinate with medical director, psychiatrists and other prescribing staff, administrative staff, clinical supervisors, Director of Community Services to continuously improve care of individuals receiving services.
  • Participate in OMHSAS and CBH required training, meetings, and clinical supervision to improve professional skills, maintain training compliance and maintain service standards as well as clinic administrative functions.
  • Provide regular mental health outpatient (MHOP) services.
  • Facilitate both individual therapy sessions, family and couples therapy sessions, and psychotherapy/psychoeducational group therapy sessions using evidenced based practices.
  • Document each session in the PIR's Electronic Health Record (EHR) within 24 hours of date of service.
  • Complete intake and other administrative forms as required within the required timelines.
  • Work in collaboration with the PIR to complete the comprehensive biopsychosocial assessment.
  • Review intake admission forms on each PIR to verify that all documentation and data are complete and in compliance with APM, city, state, and federal regulations. Obtains any missing data throughout the treatment process to ensure compliance and success of the therapeutic effort.
  • Participate with the PIR in the initial and ongoing development of recovery plan goals and objectives ensuring the plan is completed within the appropriate timeframes outlined in the program guidelines.
  • Work in collaboration with the Psychiatrist and other external multidisciplinary team members to ensure a continuity of care.
  • Ensure that all recovery plans are observable and measurable, using PIR centered language.
  • Enforce rules and regulations of the program, where applicable.
  • Make referrals to appropriate levels of care based on clinical observations of PIR's condition.
  • Maintain strong interagency community, multidisciplinary and government agencies relationships.
  • Maintain ongoing communication with other community treatment facilities, primary care physicians, hospitals, schools, and multidisciplinary units, etc. as necessary.
  • Participate in meetings with the agency interdisciplinary team for case consultations as clinically necessary.
  • Document the results of the case consultations and implements the recommended therapeutic changes to each PIR case.
  • Attend Agency meetings as required.
  • Attend and participate in scheduled in-service training, seminars, workshops or conferences while maintaining the state required 10 training hours per year.
  • Update and review PIR charts daily.
Requirements:
  • Master's Degree from an accredited university in a clinical mental health discipline, such as: psychology, counseling, clinical social work, psychiatric nursing, and marriage and family therapy.
  • Bilingual (English/Spanish) is a plus
  • Proficiency in the DAP documentation. Clinical formulations and treatment planning
  • Proficiency in the use of Microsoft Word and EHR systems
  • Experience in facilitating psychotherapeutic and educational groups
  • Motivational interviewing skills, training in Trauma, training in evidenced based practices is a plus
  • Working with medication managed populations
  • Strong verbal and written communication skills
  • Must meet all regulatory compliance requirements
  • Trained in an Evidence Based Practice recognized by CBH. APM currently offers two: Cognitive Behavioral Therapy (CBT) and Prolonged Exposure (PE).
