Bachelor's Degree in Social Service field is preferred or a minimum of five (5) years of behavioral health experience and one (1) year of direct experience may replace Bachelor's DegreeHigh school diploma or GED from an accredited institution is required Minimum of one (1) year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients; preferredExcellent customer service skillsExcellent computer skillsKnowledge of Arizona Department of Health Services and RBHA requirementsThorough knowledge of current versions of DSM and ICDAbility to complete a Behavioral Health Assessment with appropriate clinical oversightAbility to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practicesCertification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red CrossBilingual English/Spanish strongly preferred Valid Arizona driver's licenseValid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety POSITION RESPONSIBILITIES/

ESSENTIAL:


Participate as directed in the screening of new service requests within the center ensuring that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and proceduresParticipate in regular interdisciplinary staff meetingsAccept newly assigned cases in which the diagnostic intake has been completed by the Intake Specialist.

Completes case management assessment on all clients entering caseloadDevelop appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clientsProvide direct client services to individuals and/or family members.

Services provided may include case management, skills training, group counseling, group skills training, health promotion, crisis intervention, transportation, supportive employment services, counseling with appropriate clinical oversight and others depending on client need.

Services may be provided in office, home, school or community settingsProvide on-going supportive and/or case management functions in accordance with the problems, needs and the strategies identified within the service plan in order to assist the client to achieve goals and objectivesProvide face to face reviews with the client and/or relevant behavioral health staff on a regular basis.

Ensures progress made is reported so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved.

The frequency reviews will be determined by relevant requirementsAssist supervisory staff with tracking of clients, including the generation of reports that will allow follow-through on quality of careDocument all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with internal and regulatory standards requirementsMaintain close communication with the consulting psychiatrist and the Behavioral Health team for input regarding medication compliance, side effects of medication, and medication changes and alerts behavioral health team of any changes in client adjustment which might suggest decompensation and a need for more aggressive interventionPerform other duties as assigned by the supervisor which are consistent with the position and in compliance with Adelante Health Care policies and procedures

While performing the duties of this job, the employee may be required to sit for long periods of time, is required to stand, walk, use hands to handle or feel objects, tools or controls; reach with hands and arms; climb steps/stairs; balance; stoop, kneel, crouch or crawl; talk or hear; smell; manage stress as it relates to essential job functions.

The employee must frequently lift and/or move up to 25 pounds without assistance and may occasionally be required to lift or move up to 50 pounds with assistance.

Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.


Work environment:

While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites.

The noise level in the work environment is usually moderate.

The employee may be subject to health hazards (contagious diseases, blood borne pathogens, etc.) when working in the clinic area.
