The Patient Care Coordinator III plays a crucial role in providing top-tier clinical coordination and facilitation services to ensure the healthcare needs of patients are met efficiently within the system. This position requires excellent organizational skills, effective communication abilities both in person and through various electronic channels, proficiency in Call Center Operations, medical knowledge, and a deep understanding of hospital policies and procedures.

Job Responsibilities:
  • Responsible for handling calls and providing intake services on Specialty Lines.
  • Interprets patient conditions and diagnoses to direct them to the appropriate sub-specialty service.
  • Coordinates with healthcare professionals to ensure smooth admission processes.
  • Records and analyzes various documents related to service requests.
  • Communicates with facility staff to address call inquiries.
  • Maintains accurate databases related to their service line.
  • Inputs service calls into the system when necessary.
  • Manages patient information in Electronic Health Records.
  • Documents daily activities and reports them using appropriate formats.
  • Notifies management of any issues and feedback received promptly.
  • Generates performance reports through software tools.
Additional Requirements:
  • Experience: Minimum of 2 years in call center or customer service roles.
  • Education: High School diploma or equivalent.
