Social Worker

The Social Worker will provide vital services to help residents adjust to nursing home placement and ensure compliance with regulatory standards.
Key Responsibilities:
Assessment and Counseling:
- Assess psychosocial strengths and needs of residents.
- Develop and maintain social histories as part of residents' medical records.
- Provide counseling services to residents and families during the adjustment period and as needed.
Family and Community Liaison:
- Assist in the orientation of families to the facility.
- Attend and facilitate family conferences, discharge conferences, and patient care conferences.
- Maintain awareness of community resources and act as a liaison with social, health, and community agencies.
Discharge Planning and Advocacy:
- Coordinate discharge planning, transfers, and room changes for residents.
- Serve as an advocate for residents, including those requiring legal conservatorship, guardianship, or power of attorney.
Documentation and Compliance:
- Complete required documentation, including MDS and quarterly summaries.
- Ensure compliance with the Resident's Bill of Rights, the Patient's Self-Determination Act, and other regulations.
Committee Participation:
- Serve as a member of the admissions committee.
- Participate in other committees and attend meetings that impact resident care quality.
Education and Support:
- Develop and facilitate education and support groups as requested.
- Provide inservice education on advance directives, resident rights, and related topics.
Marketing and Public Relations:
- Assist in marketing the facility to prospective residents and their families.
- Maintain positive relationships with referral sources.
Qualifications:
- BA or BSW degree in Social Work, Psychology, or Sociology.
- Licensed by the State Board of Social Work.
- Two years of experience in geriatric social work.
- Membership in MNH-SWA is encouraged.
- Strong communication skills (written and verbal) in English.
- A warm, outgoing personality with the ability to establish and maintain effective relationships.
- Sensitivity to the needs of diverse populations.
- Ability to promote the growth and development of each resident.
- Organized and able to work independently within standard policies and procedures.
- Knowledge of confidentiality regulations related to residents' medical, financial, and personal information.
Compensation:
- Pay Range: $ $33.00 per hour
