Job Description

Developmental Pathways (DP) has an opening for a Service Coordinator (SC) within the Early Intervention Department. The role involves supporting services or therapy for children in various settings. The Service Coordinator will liaise with a diverse team of professionals, caregivers, families, and natural supports selected by the individual/family. They will offer guidance to Coordinator roles and handle challenging situations in collaboration with the management team, families, and providers.

This is a full-time position with a mix of remote work and occasional visits to our headquarters office in Aurora, CO, and/or other locations as required.

Requirements

  • Bachelor's degree, OR
  • Five (5) years' experience in developmental disabilities, OR
  • A suitable combination of education and experience for the role
  • Valid Colorado driver's license and proof of vehicle insurance
  • Meet driving criteria - no suspensions and maximum two (2) tickets in the last three (3) years

Preferred

  • Experience with Child Find and/or Part C laws and regulations
  • Understanding of the developmental disability system and regulations

Additional Responsibilities

  • Coordinate potential services/providers to meet individual needs as per the plan
  • Develop and review plans with families and a multidisciplinary team
  • Organize and facilitate meetings with individuals and families
  • Counsel individuals and families to establish service priorities
  • Ensure adherence to relevant rules, laws, and agency policies
  • Attend and present at community events to share program information
  • Document case notes and data entry as per expectations
  • Act as a liaison with various stakeholders
  • Problem-solve to enhance revenue for services
  • Utilize knowledge of regulations for diverse populations
