We are looking for a caring and independent PRN Home Health Registered Nurse (RN) to join our committed healthcare team at Boundless Home Health. As a PRN Home Health RN, you'll provide top-notch nursing care tailored to patients in their own homes. Your tasks will include assessing patients' health needs, devising care plans, giving medication, and educating patients and families on managing their health conditions. Your role plays a vital part in helping patients stay independent and enhance their overall quality of life.


RESPONSIBILITIES
  • Conduct comprehensive assessments of patients' health and develop personalized care plans in consultation with the healthcare team.
  • Administer medications, treatments, and wound care as per the physician's instructions.
  • Monitor patients' vital signs and symptoms, reporting any changes promptly to the physician.
  • Educate patients and families on proper healthcare strategies, disease prevention, and medication management.
  • Collaborate with healthcare professionals like LVN's and therapists for holistic patient care.
  • Effectively coordinate and communicate with the interdisciplinary team, physicians, and other healthcare providers.
  • Accurately document all patient care activities in electronic health records.
  • Offer emotional support and encouragement to patients and families.
  • Ensure compliance with relevant laws, regulations, and agency policies.
QUALIFICATIONS & REQUIREMENTS
  • Graduate of a recognized nursing school with a valid RN license from the Texas State Board of Nurse Examiners.
  • Minimum of 1 year of clinical nursing experience preferred, ideally in home health or a related field.
  • Demonstrated clinical assessment and critical thinking skills.
  • Knowledge of infusion therapy and wound care is a plus.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage time efficiently.
  • Proficiency in electronic health records and healthcare software.
  • Compassionate, empathetic, and patient-centered care approach.
  • Reliable transportation and valid driver's license.
  • Current Basic Life Support (BLS) certification.
