Job Description:
1. Patient Assessment and Care Planning:
  • Conduct comprehensive assessments of patients' health status, including vital signs, medication management, wound care, and other nursing interventions.
  • Develop individualized care plans in collaboration with patients, families, and healthcare providers to address specific health needs and goals.
2. Skilled Nursing Care:
  • Administer medications and treatments as prescribed by healthcare providers, ensuring adherence to safety protocols and regulatory guidelines.
  • Provide skilled nursing services such as wound care, catheter care, tracheostomy care, and diabetic management as needed.
  • Monitor patients' progress and response to treatment, adjusting care plans as necessary to optimize outcomes.
3. Patient Education and Support:
  • Educate patients and their families about their medical conditions, treatment plans, and self-care techniques to promote independence and optimal health outcomes.
  • Provide emotional support and counseling to patients and families, addressing concerns and facilitating coping strategies.
4. Collaborative Care Coordination:
  • Communicate effectively with interdisciplinary healthcare teams, including physicians, therapists, social workers, and home health aides, to ensure coordinated and holistic care delivery.
  • Participate in care conferences and interdisciplinary meetings to discuss patient progress, goals, and care planning strategies.
5. Documentation and Reporting:
  • Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and progress notes in electronic health records (EHR) systems.
  • Report any changes in patients' condition or significant events to the supervising registered nurse (RN) or healthcare provider in a timely manner.
6. Safety and Infection Control:
  • Adhere to infection control protocols and safety standards to minimize the risk of healthcare-associated infections and ensure a safe environment for patients and caregivers.
  • Follow established procedures for handling and disposing of medical waste and hazardous materials.
