We are looking for a dynamic bilingual Medicare Care Coordinator to join our team. In this role, you will work independently to ensure high-quality, cost-effective patient care for members requiring coordination. Responsibilities include reviewing medication profiles, coordinating office visits, and collaborating with healthcare providers across different specialties.

The ideal candidate will conduct member outreach, educate them on healthcare needs, and utilize community resources to optimize care coordination and health outcomes. Additionally, you will collaborate with various teams within Jefferson Health Plans to enhance preventive health practices and screenings.

As a Medicare Care Coordinator, your daily tasks will involve:

  • Conducting proactive member outreach through calls or home visits
  • Developing personalized Care Plans focusing on the member's goals and barriers to care
  • Documenting care plans and follow-up schedules accurately
  • Coordinating care with community resources and behavioral health providers
  • Informing members about Medicare benefits and sending relevant literature
  • Collaborating with internal and external healthcare partners to meet member needs
  • Tracking cases for timely interventions and assessing outcomes
  • Participating in care coordination program evaluations and improvement initiatives
  • Acting as a liaison for members within Jefferson Health Plans
  • Assisting in quality care and utilization management

Qualifications:

  • Licensed Social Worker (LSW), Registered Nurse (RN), or Licensed Practical Nurse (LPN)
  • Fluent in Spanish with effective communication skills
  • Detail-oriented, organized, and proficient in handling multiple tasks
  • Excellent verbal and written communication skills
  • Computer literate with proficiency in Microsoft Word, Excel, and Outlook
  • Understanding of Medicare Managed Care systems
