About the Role:
  • Work collaboratively with physicians to create personalized care and treatment plans tailored to clients' home environments, focusing on fall prevention, interventions, and home safety.
  • Monitor and communicate clients' response to treatment or changes in condition to the Director of Nursing and physicians.
  • Implement physical therapy programs and educate other team members, family members, and caregivers on assisting with clients' physical therapy needs.
  • Prepare detailed clinical summaries and progress reports, and actively participate in discharge planning processes.
  • Show professionalism and empathy towards clients, families, and colleagues at all times.
Requirements:
  • Hold a master's degree or higher in physical therapy from an accredited program.
  • Successfully passed the National Physical Therapy Examination (NPTE) and any state-specific exams.
  • Prior experience in community or home health settings is a plus.
