Provides pharmaceutical services to patients receiving care in post-acute, long term, or retail settings in a manner that delivers superior patient care and excellent customer service.

Monitors and manages the prescription process and daily activities of the closed-door long-term care pharmacy in accordance with company, state and federal policies, laws and regulations.

Document all communication with physicians, nurses, and other members of the healthcare team.· Take responsibility for accurate intake and efficient fulfillment of prescription orders.· Obtain, as necessary, and maintain all required documentation in accordance with federal and state regulatory agencies and Pharmacy Services policy.· Provide and document interim Medication Regimen Reviews for all new admit or readmit patients.· Implement and document all approved therapeutic substitution medication orders.· Document and investigate errors and variances.· Take ownership and accountability for the successful completion of all pharmacy production duties and achievement of pharmacy location goals.


Graduate of ACPE accredited School of Pharmacy.· Long-term care pharmacy experience preferred.· Experience in patient care and medication management, drug therapy monitoring, drug and medical supply procurement, inventory control management, and performance improvement.· As an Equal Employment Opportunity employer, all qualified applicants will receive consideration without regard to race, color, religion, sex, national origin, disability, or veteran status.
