About the Nurse Practitioner Role at our Passionate Healthcare Company

We are in search of a dedicated Nurse Practitioner to join our team focused on enhancing the well-being of our patients. As a member of the Quality Department, you will be part of the Clinical Operations team working towards enhancing health outcomes by organizing and providing essential clinical care. Through education and outreach initiatives, we aim to motivate patients to adopt a preventive approach to managing their health and taking an active role in handling chronic illnesses. Our ultimate goal is to promote healthy living at all life stages.

Responsibilities:

  • Conduct 10-12 Annual Wellness Visits daily
  • Document patient history comprehensively
  • Identify care gaps and recommend necessary screenings or diagnostics
  • Create personalized prevention plans based on patients' health status
  • Offer patient education on preventive services and treatment
  • Make referrals to primary care physicians and relevant services
  • Collaborate with the AWV Care Coordination team
  • Ensure accurate and timely documentation
  • Stay updated on HCC and HEDIS measures
  • Monitor and comply with legal requirements

Qualifications:

  • Active Nurse Practitioner License and Furnishing License
  • Current Board Certification with recognized bodies
  • Valid DEA and NPI
  • Location in California, Texas, Arizona, or Nevada

Desired Skills and Experience:

  • At least one year of direct patient care experience in relevant fields
  • Understanding of HEDIS and HCC in relation to AWV/Medicare demographic
  • Proficiency in Spanish or Mandarin
  • Familiarity with Electronic Health Records
