Job Overview

Evaluates, plans, directs and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability.

Writes evaluations, progress notes/reports, discharge summaries and maintains patient files. Counsels patients and/or family members regarding Rehabilitation diagnosis, plan of care and prognosis. Maintains current knowledge of regulatory/legislative policies as they apply to the profession of Physical Therapy and adheres to same.

Qualifications

Graduate of Accredited Physical Therapy Program/Occupational Therapy Program,, Current NJ State License in Physical Therapy plus a minimum of five years previous work related experience.

CPR certification within first 3 months of hire.

FIM certification to be taken following employment and training at Wayne, then certification must be taken through the rehab program (Uniform Data System for Medical Rehab-UDSmr).

Specialty certification in area approved APTA are of practice or as deemed appropriate by the leadership staff (ie:
CHT- Certified Hand Therapist, OCS-Orthopedic Certified Specialist, PCS-Pediatric Certified Specialist, NDT-NeuroDevelopmentalTechnique) .
